We are committed to conducting our business in accordance with the Australian Privacy Principles in order to ensure that the confidentiality of personal information is protected and maintained.
During your time at Waverley Endoscopy we will obtain information pertaining to you which we utilise from either a medical or a financial point of view. We do take all reasonable precautions to ensure that the personal information we collect is accurate, complete and up to date. In this endeavour we do rely on the information given to us being accurate.
Type of information Waverley Endoscopy holds about you
Personal information that we collect from you are your current contact details such as address, phone number, marital status, next of kin and country of birth. These are usually asked for on booking of appointment or on admission to the hospital.
Medical information required by us to ensure safe care of you during your visit includes your medical history (problems, medications and allergies).
Financial information and health insurance details are requested so that we may process your accounts.
How we collect the information
Waverley Endoscopy collects personal information in a number of ways, including:
- Directly from you, when you provide information by phone or in documents such as consent form.
- From third parties such as our related companies, Medicare and health funds.
- Your GP will provide a medical referral containing past and present medical details.
How we use your personal information
- To provide the medical attention you require.
- Administer and manage financial services including charging and billing.
- Reporting of mandatory health data to both Commonwealth and State Governments.
Protection of your personal information
Waverley Endoscopy stores and maintains your personal information according to Australian Standards.
All paper records are scanned into our computers securely with access restricted to the relevant Waverley Endoscopy staff.
Electronic information is accessed using a secure password.
All patient records containing personal information or identifying material is dealt with in a confidential manner except where necessary to enable another health worker to assist in the patient’s care, or when authorised by, or under law.
